Custom Forms Overview:

How to Send a Custom Form:

  • Navigate to patient page in Athena → click hamburger button → select “send patient forms” →  select forms from the dropdown → click "send patient forms"

What is Pre-Visit History?:

  • Users can now see a snapshot of the patient’s pre visit status as well as all steps they have completed and outstanding requirements before their check-in is complete 

PreVisit Workflow:

  • Staff can see if patients have any outstanding requirements and have the patient complete requirements upon arrival on their mobile device or the in-office iPads 

How to See a Full PreVisit History:

  • Navigate to the patient tab of the Check-In extension → find "PreVisit status" section → click "PreVisit history"

Text Message History:

  • Navigate to patient tab of Check-In extension → find “send patient text section” → click on “see text message history”

What are Custom Alert Notifications?:

  • Custom alert notifications are non-dismissable windows of info that can be customized to be shown at top of any step in pre-visit check-in

  • Error alert, info alert, success alert, warning alert
