Custom Forms Overview:
- Custom forms allow practices to set up forms/consents for patients to complete during check-in process
Document Type and Status:
- Custom forms can be made within the following document types: admin, encounter, or clinical
- Forms can be in REVIEW status (require review by the clinical provider) or CLOSED status (do not require review)
Configurations:
- Forms can be configured by patients age, sex, provider specialty, department, appointment type
- They be configured to expire within given time frame, causing the patient to receive the form again once the time has elapsed
Signing Custom Forms:
- Any form that requires a signature has a drop down menu with who is signing on behalf of the patient
Guidelines for Sending Forms:
- Clinical documents and admin documents can be sent before, during, or after appointment
- Encounter documents can only be send before or during an appointment
How to Send a Custom Form:
- Navigate to patient page in Athena → click hamburger button → select “send patient forms” → select forms from the dropdown → click "send patient forms"
What is Pre-Visit History?:
- Users can now see a snapshot of the patient’s pre visit status as well as all steps they have completed and outstanding requirements before their check-in is complete
PreVisit Workflow:
- Staff can see if patients have any outstanding requirements and have the patient complete requirements upon arrival on their mobile device or the in-office iPads
How to See a Full PreVisit History:
- Navigate to the patient tab of the Check-In extension → find "PreVisit status" section → click "PreVisit history"
Text Message History:
- Navigate to patient tab of Check-In extension → find “send patient text section” → click on “see text message history”
What are Custom Alert Notifications?:
- Custom alert notifications are non-dismissable windows of info that can be customized to be shown at top of any step in pre-visit check-in
The 4 Types of Customer Alerts:
- Error alert, info alert, success alert, warning alert
